Q: What do I need to bring to my therapy appointment?

A:   1. Health insurance card (and Vehicle insurance card if involved
           in a motor vehicle accident).
       2. Prescription from your physician
       3. Depending on the diagnosis, you may want to wear loose
fitting clothes or bring shorts and a t-shirt.

Q: I have a question regarding a therapy bill I received.
     Who do I contact?
A:    Barry Bottger at 785-527-6050

Q: Do I have a co-pay at the time of service?
A:     This will depend on the type of insurance you carry.
We should be able to tell you by the end of your first
         appointment.

Q: Do I need to see my physician before I can receive therapy?
A:     Yes. We need a referral from your primary care or specialty

The exception to this would be if you have been treated for the
same condition within the calendar year you do not need a referral
         from a Physician.

Q: How long is each therapy appointment?
A:     Most appointments will last 30 to 60 minutes.

Q: How often do I have to come to therapy?
A:     The therapist will be able to tell you after your first
appointment. Typically Patients are seen 2-3 days a week.

Q: What are your hours?
 A:     8:00 a.m. to 5:00 p.m., Monday through Friday.
